Worker's Comp Patient Information
If you are injured on the job or in the course of your employment, you may be eligible for your care to be paid by your employer's Worker's Compensation Insurance carrier.
You must first notify your employer of your injuries. They will then contact their Worker's Comp insurance carrier who will then authorize treatment and advise you as to which doctor(s) participate with them. If we are not on the list of participating providers for that carrier and you wish to treat at our office, you may request that we be added.
In order to begin treatment at our office, you will need to provide us with an accident report, an authorization number from your employer/ Worker's Comp. carrier and the Worker's Comp. carrier contact information.
